Clinical Site Director - Elder Service Plan

About the role

Oversee day-to-day clinical and site operations for the Elder Service Plan PACE center in Brockton. Manage interdisciplinary care delivery, staffing, clinical quality, regulatory compliance, budgets, workflows, and services supporting elderly program participants.

What you'll do

• Oversee hiring, training, development, supervision, and evaluation of staff
• Manage interdisciplinary team operations and ensure participant goals guide care decisions
• Oversee clinical services, phone triage, care coordination, nutrition, social work, recreation, and site operations
• Develop and implement standard operating procedures, workflows, and quality improvement initiatives
• Monitor compliance with clinical standards and local, state, and federal regulations
• Ensure adequate staffing, supplies, on-call coverage, and continuity of operations
• Coordinate home health, rehabilitation, long-term care, and specialty services
• Assist with operational and capital budgets and manage services against financial benchmarks
• Maintain CMS, OSHA, and other applicable compliance requirements

What you'll bring

• Bachelor's degree in a healthcare or human services discipline
• Current Massachusetts licensure in a clinical discipline
• Current Basic Life Support certification for healthcare providers
• Three to five years of progressively responsible management and staff supervision experience
• One to two years of healthcare experience, including at least one year in geriatric care
• Knowledge of the physical, mental, and social needs of elderly adults
• Effective leadership, decision-making, planning, communication, and group facilitation skills
• Intermediate proficiency with Microsoft Office 365 and electronic medical records
• Registered Nurse licensure strongly preferred
• Knowledge of PACE, quality improvement, care management, and Massachusetts healthcare networks preferred
