Responsibilities:Provide direct patient care to senior clients in a home care setting under the supervision of a Registered Nurse or physicianAdminister medications, treatments, and injections as prescribed and document accordinglyMonitor and record patient vital signs, health status, and changes in conditionAssist with wound care, catheter care, and other skilled nursing proceduresCommunicate effectively with patients, families, and the care team regarding patient progress and needsMaintain accurate and timely clinical documentation in compliance with state and agency regulationsEducate patients and caregivers on health management, medications, and care plansRequirements:Active Licensed Practical Nurse (LPN) license in the state of VirginiaPrevious home care, geriatric, or long-term care nursing experience preferredCurrent CPR/BLS certificationStrong clinical assessment and critical thinking skillsExcellent communication and interpersonal skills with a patient-centered approachReliable transportation and valid driver's license for home visitsAbility to work independently with minimal supervision in a home care environmentAbout Us:North Point Senior Homecare is a trusted provider of in-home care services for seniors in the Virginia Beach area, dedicated to helping older adults live safely and comfortably in their own homes.
