RN MDS Coordinator

About the role

Coordinate MDS assessments and care plans for residents of Evergreen Living Center. This non-union nursing leadership position has a flexible Monday-Friday schedule of either five 8-hour shifts or four 10-hour shifts, includes on-call rotations and one weekend per month.

What you'll do

• Coordinate clinical MDS assessments for all Evergreen residents
• Ensure department heads complete assigned assessment sections accurately and on time
• Create and distribute assessment schedules to appropriate staff
• Coordinate the completion, updating, and revision of resident care plans
• Participate in resident care plan meetings and document summaries in medical records
• Work with CNAs to improve documentation accuracy and provide education and assistance
• Participate in nursing leadership and on-call rotations
• Work one weekend per month
• Assist the facility with additional projects and responsibilities as needed
