Social Worker (LMSW)

About the role

Jan Werner Adult Day Care Center seeks a licensed master's social worker to support adult day care participants and their families in Potter County, Texas. Reporting to the Social Services Director, this onsite role serves as a participant advocate and connects individuals with community resources that address psychosocial needs. You will guide prospective participants through assessment and enrollment, maintain accurate records, and contribute to individualized plans of care alongside an interdisciplinary team. Responsibilities include family communication, referrals to appropriate agencies, participant rights education, and counseling as ordered by a physician and recommended by the team. The ideal candidate brings a master's degree in social work, strong documentation skills, and a compassionate approach to working with older adults. Two years of geriatric experience is preferred.

What you'll do

• Coordinate participant intake, psychosocial assessments, and enrollment follow-through.
• Develop and update individualized care plans with the interdisciplinary team.
• Maintain timely medical records, social histories, and annual participant documentation.
• Connect participants with community agencies and coordinate with assigned caseworkers.
• Educate participants, families, and staff about rights, advance directives, and available services.
• Provide authorized counseling and communicate changes affecting participant well-being.

What you'll bring

• Master's degree in social work from an accredited school.
• Current Texas Licensed Master Social Worker credential.
• Two years of experience working with geriatric populations preferred.
• Strong assessment, care coordination, and documentation skills.
• Ability to communicate effectively with participants, families, and interdisciplinary teams.
• Knowledge of community resources, participant advocacy, and confidentiality practices.
